Abstract (PL)

COVID-19 to choroba zakaźna, która rozprzestrzenia się drogą powietrzną. Jej wybuch pogrążył cały świat w nieprzewidzianym scenariuszu, pochłaniającym tysiące istnień i zmieniającym życie ludzi na całym świecie do egzystencji w warunkach pandemii. Ze względu na szybkość rozprzestrzeniania się tej choroby na całym świecie, ma ona destrukcyjny wpływ na opiekę zdrowotną, gospodarkę globalną oraz codzienne czynności każdego z nas. W związku z tym, niezwykle ważne jest jak najszybsze wykrycie i zdiagnozowanie COVID-19. Rozpoznanie tej choroby można przeprowadzić za pomocą takich metod obrazowania, jak RT-PCR i CT klatki piersiowej. Chociaż testy RT-PCR są czułe i dość szybkie, są narażone na wyniki fałszywie ujemne i fałszywie dodatnie. Badania rentgenowskie klatki piersiowej odgrywają ważną rolę we wczesnym diagnozowaniu chorych pacjentów. W przypadku obrazowania CT, pomimo ewidentnych korzyści w wykrywaniu choroby, mogą wystąpić błędne rozpoznania pomiędzy obrazami zawierającymi cechy COVID-19 oraz cechy innymi chorób. W związku z tym, niezbędna jest szybka i dokładna identyfikacja pacjentów chorych, aby zapobiec źródłom infekcji. Dlatego też, trwają poszukiwania nowych metod diagnozy, opartych zwłaszcza na uczeniu maszynowym. Badania przedstawione w niniejszej rozprawie koncentrują się na rozwiązaniu zadania klasyfikacji, ponieważ, zdaniem autorki, najlepiej nadaje się ona do diagnostyki COVID-19. Na podstawie materiału badawczego w postaci danych klinicznych i demograficznych 128 pacjentów (77 pacjentów z COVID-19 i 51 z zapaleniem płuc wykazano, że można zbudować model uzyskujący bardzo dobre wyniki klasyfikacji, przekraczające poziom 80% dla takich miar jakości, jak dokładność (ACC), czułość (TPR) i swoistość (TNR). W rozprawie zaproponowano dwa modele klasyfikacyjne: RFE-RF-15 i E-ADA-4. Modele te uzyskały najlepsze wartości metryk jakości klasyfikacji (dokładność, czułość, swoistość, AUC), dlatego można je rekomendować do wykorzystania w systemie wspierającym lekarza w diagnostyce COVID-19. W końcowej części pracy przedstawiono plan dalszych badań w kierunku poprawy jakości diagnostyki choroby z wykorzystaniem wyników przedstawionych w niniejszej rozprawie.

Abstract (EN)

COVID-19 is an infectious disease that spreads through the air. The COVID-19 outbreak has plunged the whole world into an unforeseen unpleasant scenario, bringing the lives of people around the world to the level of a pandemic and claiming thousands of lives. Due to the rapid spread of COVID-19 around the world, it is having a devastating impact on issues such as healthcare, the global economy and daily activities. In this regard, it is extremely important to detect and diagnose COVID-19 in patients as soon as possible. The diagnosis of COVID-19 can be achieved using Real-Time reverse transcriptase Polymerase Chain Reaction (RT-PCR) and Chest CT imaging scan methodologies. Although RT-PCR tests are sensitive, fairly fast, and reliable, these tests are at risk for false negatives and false positives. Chest x-rays such as computed tomography (CT) and x-rays play an important role in the early detection and management of patients with COVID-19. Despite the benefits of CT imaging in identifying patients with COVID-19, misclassification between COVID-19 imaging features and other disease types may occur. To this end, rapid and accurate identification of patients with COVID-19 is essential to prevent sources of infection. Therefore, the search is underway for new methods of diagnosis of COVID-19, especially based on machine learning. In this dissertation, research is focused on the classification task, as it is best suited for the task of predicting the disease of COVID-19. Based on the study material in the form clinical and demographic data of 128 patients (77 COVID-19 patients and 51 pneumonia (Non-COVID-19 patients)), it is shown that with the right choice of model, very good classification results can be achieved, exceeding 80% in terms of accuracy, sensitivity (TPR) and specificity (TNR). Two classification models were proposed in the dissertation: RFE-RF-15 and E-ADA-4, which help the doctor in the diagnosis of the infectious disease COVID-19, since these models showed the best results in classification parameters. Evaluation metrics of models as, accuracy, sensitivity, specificity and AUC were also assessed. The final part of the paper presents proposals for further research in the direction of classification of COVID-19 diagnostics based on the results presented in the dissertation.
