W rozprawie doktorskiej przedstawiono wyniki badań eksperymentalnych i analizy biomechaniki sił działających w gorsetach ortopedycznych typu Chêneau u pacjentów chorych na młodzieńczą skoliozę idiopatyczną. W pierwszej części rozprawy opisano medyczne aspekty leczenia skolioz oraz biomechanikę sił z jakimi sztywne ortezy oddziałują na tkanki pacjenta. Dokonano również szerokiego przeglądu stanu nauki dotyczącego terapii aktywnymi gorsetami ortopedycznymi. Przedstawiono zalety stosowania takich ortez w porównaniu do ich statycznych odpowiedników. W drugiej części przedstawiono wyniki prac badawczych wykonanych w ramach doktoratu. Opisano techniczne aspekty akwizycji danych wraz z opisem działania systemu i metod przetwarzania zebranych danych. Omówiono sposób przechowywania i organizacji danych. Przedstawiono kryteria klasyfikowania pacjentów oraz szczegółową analizę wyników pomiarów sił nacisku, zarówno w kontekście aktywności dobowej, jak i z dłuższego okresu czasu. Na podstawie wyników pomiarów oraz ich analizy wyznaczono parametr regularności noszenia gorsetu odnoszący się bezpośrednio do systematyczności i stosowania się do zaleceń lekarza w kwestii czasu noszenia ortezy w ciągu doby. Dokonano klasyfikacji danych pomiarowych wykorzystując statystyczne modele klasyfikujące ze skutecznością wynosząca˛ 99,95%. Pozwoliło to na opracowanie systemu decyzyjnego wspomaganego technologiami Internetu Rzeczy, uczenia maszynowego i sztucznej inteligencji, umożliwiającego zdalny monitoring i optymalizację sił działających w ortopedycznym gorsecie typu Chêneau. Uzyskane wyniki prac badawczych i przeprowadzonych analiz zapewniają gwarancję efektywnego wdrożenia systemów decyzyjnych i samouczących do protokołu leczniczego skolioz, ukierunkowanego na indywidualne potrzeby każdego pacjenta.
This dissertation presents the results of an experimental study and analysis of the biomechanics of the forces in Chêneau-type orthopaedic braces during adolescent idiopathic scoliosis treatment. The first part of the dissertation describes the medical aspects of scoliosis treatment and the biomechanics of the forces that rigid orthoses interact with the patient’s tissues. The actual knowledge regarding active orthotic brace therapy is also broadly reviewed. The advantages of using such orthoses compared to their static counterparts are presented. The second part presents the results of the research work carried out as part of the PhD thesis. The technical aspects of data acquisition are described, together with a description of the system operation and the methods of processing the collected data. The method of data storage and organisation is discussed. Criteria for the classification of patients and a detailed analysis of the results of pressure force measurements, both in the context of daily activity and over a longer period of time, are presented. On the basis of the measurement results and their analysis, a brace-wearing regularity parameter was determined that relates directly to the regularity and adherence to the doctor’s recommendations regarding the time of wearing the orthosis per day. A classification of the measurement data was carried out using statistical classification models with a 99.95% success rate. This allowed the development of a decision-making system supported by Internet of Things, machine learning and artificial intelligence technologies to remotely monitor and optimise the forces in a Chêneau-type orthopaedic brace. The obtained results of the research work and the analyses provide a guarantee of effective implementation of decision-making and self learning systems into the scoliosis treatment protocol, targeted to the individual needs of each patient.
